Long-Term Care Admissions Feedback Survey

Moving into long-term care is a stressful experience for the new resident and their family. Decisions need to be made quickly and there is so much information to learn- “what can I bring on admission day? What does 24/7 nursing care even mean?” It’s a whole new world to get to understand and find your place in! The transition process is both difficult for families and can strongly impact their readiness and ability to become effective Family Council members. Having the right information at the right time can help make that transition a little less stressful and FCO is here to help!

To help families adjust better and more quickly to being a part of the LTC community and becoming an effective Family Council member, we’re developing a series of educational materials on everything you need to know and didn’t know that you needed to know about long-term care. The goal of these guides is to provide information to help families better prepare for and understand the transition into long-term care- before they even step into the home.

Making the decision to transfer a loved one into long-term care is a difficult one. You can help FCO to develop materials that will provide prospective and new LTC family members with the need-to-know information that will help them prepare for the reality of LTC and their new role as a Family Council member. Complete our survey to share your thoughts on what you wish you had known before becoming a LTC caregiver. Based on your feedback, we’ll develop materials and resources that speak directly to your experience and answer questions that new LTC families don’t even know they have.

About the Survey

Nous vous demandons de prendre 10 minutes de votre temps pour répondre à un sondage afin de partager vos réflexions sur votre expérience de soutien à un être cher lors de sa transition dans un établissement de soins de longue durée. Ce projet est dirigé par Sam Peck, Liane Pelissier, Cathleen Edwards (équipe des Conseils de la famille de l'Ontario) et soutenu par Carol Lemen et d'autres étudiants en travail social effectuant leur stage à Family Councils Ontario. Seuls les membres de l'équipe du projet auront accès aux données du sondage. Si vous répondez au sondage, vos données numériques seront gardées en toute sécurité sur le OneDrive de Family Councils Ontario pour une période de 7 ans.
