Primary Interface w Interdisciplinary Health Team Promotion: Aging In Place

This presentation is the kickoff session of a 3-part series on Aging In Place, focusing on Interdisciplinary Partnerships.

About this event

Institute on Aging welcomes Karen Kwok, Family Nurse Practitioner, Hartford Academy Geriatric Interprofessional, as she leads a 3-part "Virtual Briefcase Workshop Series" throughout May, 2022, during which service organizations, practitioners, patients and their caregivers can share their successes and challenges with aging well in place.

As a seasoned primary care provider, Ms. Kwok will utilize a didactic approach with lecture content; case scenarios to illustrate learning objectives; and audience input---all contributing to rich discussion.

This first session will be a kickoff to the series, in which the following topics will be explored:

○ Recommendations on effective partnership with primary care

○ Understanding how primary care advocates/communicates for your interdisciplinary needs

○ Long-term planning for the changing body and evolving physical and behavioral needs

○ Resources for aging well in place: legal (DNR, DNI, DPA), caregiver respite, ensuring patients are safe and families are well-supported in their caregiving.

By the end of this training, participants will be able to:

o Identify at least 3 strategies for ensuring that primary care fosters effective partnerships for patient care

o Create an outline of needs illustrating how primary care can advocate for an effective interdisciplinary approach

o Adapt expectations across the lifespan for evolving physical and behavioral health needs

o Connect with at least 3 resources for aging well in place: legal (DNR, DNI, DPA), caregiver respite, patient safety

Your Presenter: Karen Kwok, Family Nurse Practitioner-Board Certified, Master of Public Health, Master of Science in Nursing

Karen Kwok, MSN, FNP-BC, MPH, is an advocate, educator, and collaborator in public health and family nursing. With over 20 years committed to patient health advocacy and health equity, she serves as a Nurse Practitioner in the local DC, MD, and VA communities. Karen’s areas of scholarship include improving access to primary health care and community-based partnership development with recognition from American Nurses Credentialing Center, Robert Wood Johnson, Bill and Melinda Gates Institute, California Governor Gray Davis, President Barack Obama, and Fulbright Specialist, for her service. Her contributions to domestic and global health initiatives include capacity development, strategic planning, and program evaluation with Ministries of Health spanning Latin America, Central and Southeast Asia, and Sub-Saharan Africa and diverse US communities including Asian American Native Hawaiian Pacific Islanders, Latinx, African and African Americans, and American Indian and Alaskan Natives. She consults for a variety of organizations and in a variety of roles, ranging from companies working on innovative point-of-care medical devices, fiscal sponsoring agencies promoting the public welfare of children and seniors, objective reviewing for nonprofit grant applications, and capacity building with private and governmental contractors. As a former geriatric fellow with the Hartford Academy, Karen is enthusiastic about advancing the discussion on aging well in place.

All Continuing Education certificates will be delivered digitally, within 14 working days of the event. After the program, an online survey will be offered for those who wish to receive CEs.

Registration Ends on May 10, at 12:25 p.m.

This class will be appropriate for those in aging services, including: family therapists; social workers; nurses; elder-law practitioners; long-term care administrators; gerontologists, human resources professionals and others. But it will also be very helpful and of interest to the lay person. No previous knowledge of the content area is required.
