Who will make healthcare decisions for you when you are unable and are they equipped to honour your wishes?

Join us Monday, May 16, 2022 from 4:00PM to 6:00PM via Zoom for an Advance Care Planning and Substitute Decision Maker Information Session. This is important and helpful information for anyone at any age. In this session we will:

• Determine who your substitute decision maker is (everyone in Ontario has one)
• Learn how to ensure they understand your goals, values, and wishes so you feel they are able to speak for you in the event you are unable.
• Discuss what you need to know and understand if you are to speak on the behalf of loved ones as a substitute decision maker.

Please share with anyone who you feel may be interested in attending. This is open to clients, volunteers, staff, and community members.
