We will practice having Advance Care Planning conversations and explore a variety of tools and games to help us have these important conversations with loved ones.

This session requires prior Advance Care knowledge. If you have not had a chance to attend one of our Advance Care Planning Information Sessions, please visit the HPCO’s Advance Care Planning website and work through the information provided there to prepare.
