From Hospital to Home for Persons Living with Dementia: A Multi-Perspective Qualitative Study,

A research study, “From Hospital to Home for Persons Living with Dementia: A Multi-Perspective Qualitative Study,” is being conducted by nurse and PhD student Marianne Saragosa at the University of Toronto’s Institute of Health Policy Management and Evaluation.
She is looking for older adults living with dementia, or a caregiver of a person living with dementia, who has experienced a discharge from hospital back to their home in the community to participate in a one-time interview about that experience. This includes older adults discharged from acute care, ED, rehab, etc. COVID-19 has shed an important spotlight on the essential role of unpaid caregivers.
To help further our understanding of care transitions involving people living with dementia and their essential caregivers, please consider participating, or if you know of anyone who would be interested in participating, please provide the attached information regarding this study.
Please consider participating in an interview about your experience. During the interview, you will be asked to answer questions about:
(1) The care transition experience going from hospital to home;
(2) Any hardships that you experienced since returning home;
(3) Your thoughts on what could be done to make the transition better.
A single interview should take 45-60 minutes, with the potential of a follow-up interview to talk about initial study findings.
