MLTC Engagement on Quality: Invitation to Families/Caregivers

Dear Families/Caregivers of Residents in Long-Term Care,
The Ontario Ministry of Long-Term Care is currently developing a Quality Framework and we need your input!
The Quality Framework will outline consistent definitions of quality of life and quality of care. It will also identify the measures to assess and publicly report on the performance of LTC homes and will set the foundation to improve quality of life and quality of care for long-term care residents.
To inform this important work, the ministry will be holding several virtual engagement sessions in August/September 2021 to gather your perspective on what really matters to your loved one in their day-to-day life and about the care they receive. Please note that separate sessions are also being offered to residents in long-term care.
Whether or not you decide to participate in the Family/Caregiver Engagement Sessions, there will be no impact on your family member in long-term care or the services they receive at the long-term care home at which they reside or from other health care providers.
Each session will include about 8-10 participants, all of whom are or were family/caregivers of residents in long-term care.
You will join the session using the virtual meeting platform, Microsoft Teams.
At the session, you do not need to share your name, but if you would like to, please only use your first name. Please note if you already have a Microsoft Teams account your username will be displayed and viewable by the other participants and the ministry facilitator and will be recorded.
The sessions will be recorded and transcribed. If your camera is on during the engagement session, which we encourage, your video will be recorded too.
Participants will be asked 2-3 questions about their loved one’s experience living in a long-term care home, and what could be improved.
- § Your feedback will be anonymized and summarized into key themes to help to prioritize future initiatives.
The session will be facilitated by an external facilitator, on behalf of the Ministry of Long-Term Care. Ministry staff will also be in attendance.
If you would like to take part in one of the Family/Caregiver Engagement Sessions please complete the Registration Form and the Consent for the collection, use and disclosure of your personal information by clicking on the link below.
Participants will be asked to ensure that they do not disclose any identifying information about themselves or others during the sessions (including identifying information of their family member who live in long-term care, other family members and the long-term care home staff) beyond potentially their own name displayed in the Teams sessions.

If you consent to participate, the personal information collected in the registration form will be kept confidential and will only be used as permitted under and in accordance with your consent form and the Freedom of Information and Protection of Privacy Act. You will also be asked to select your preferred session date and time as shown below.

You will receive confirmation on the date and time of your engagement session once the registration process is complete (approximately seven days after the deadline for registration). Participants will be selected to ensure broad representation and on a first come first serve basis.

This invitation is for families and caregivers of residents in LTC. We encourage LTC staff to share this information with families in their networks and LTC communities.
