This webinar will present the results of the PoET Southwest Spread Project (PSSP), which took place between 2019 - 2022 in Southwestern Ontario. PSSP was a joint venture between William Osler Health System and the Department of Family Medicine at McMaster University, and supported through a financial contribution from the Health Care Policy Contribution Program of Health Canada.

Through PSSP, the PoET (Prevention of Error-based Transfers) Project was launched in 54 Ontario long term care homes, helping them to align their habits, policies, and practices with Ontario's Health Care Consent Act. This webinar will present findings related to the association of PoET's implementation with rates of transfer to hospital (at any time, and at end of life), and palliative care encounters. There will be time allowed for questions and answers.
